What is the difference between SN1, SN2, E1 and E2?

All four begin the same way — a substrate with a leaving group meets a nucleophile or a base. Substrate class decides most of it: methyl and primary go SN2, tertiary goes SN1 or E1 through a carbocation, and a strong bulky base forces elimination.
